KINDERGARTEN REGISTRATION

Packets will be available in the main office beginning on January 2, 2008 between the hours of 9:00AM to 3:00PM. At this time an appointment may be made for a physical exam with our school physician.

Return the completed packet on the following registration days:

Thursday, January 24: A-M (9:30-11:30)

Thursday, January 31: N-Z (9:30- 11:30)

To be eligible for enrollment a child must be 5 years old on or before October 1, 2008

REGISTRATION WILL NOT BE COMPLETED UNLESS ALL OF THE REQUIRED DOCUMENTS ARE PRESENTED.

The following items are necessary for you to present upon registering your child:

	I. Birth Certificate (Original - Vital Statistics Dept.)
	II. Proof of immunization, under doctor's signature for:

DPT Series & Booster Boosters must be done on or after 4th Birthday {4}

Polio Series & Booster Boosters must be done on or after 4^th Birthday {3}

Measles Vaccine 2

Rubella (German Measles) Vaccine 1

Mumps Vaccine { 1}

Hepatitis B Vaccine {3}

Varivax (Chickenpox) Vaccine {1}

III. Certificate of Health (report of a physical examination done by your physician between 9/1/06 and 9/1/07)

IV. Both the child and parents(s) or guardian(s) must have proof that they are borough residents.

Accepted forms of residence include:

Current tax bill or receipt

Mortgage statement

Current rental agreement or lease

Contract to purchase a home

Notarized letter from a landlord or property (accompanied by owner’s proof of residence)

Property Deed

***DRIVER'S LICENSE OR UTILITY BILL WILL NOT BE ACCEPTED
