ATHLETIC REGISTRATION INFORMATION
READ THIS SECTION IN ITS ENTIRETY
Prior to participation on a school-sponsored athletic team, every student athlete in grades 6 through 12 must complete the Athletic Registration Forms in the parent portal of Genesis (for each and every sport they participate in). The Athletic Registration Forms in the parent portal of Genesis DO NOT need to be printed. Submit them electronically through the parent portal.
In addition, prior to participation on a school-sponsored interscholastic or intramural athletic team or squad, each student-athlete in grades six through 12 must present a completed Preparticipation Physical Evaluation set of forms (PPE) to the school nurse. Important information regarding the PPE is provided below, and you should feel free to share with your child’s medical home health care provider.
- The PPE may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment professional development module. It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE.

The required PPE must be conducted within 365 days prior to the first official practice in an athletic season. PPE can be found here.

The parent/guardian must complete The Preparticipation Examination, History Form (page one), and insert the date of the required physical examination at the top of the page. The form must be completed PRIOR to the examination and presented to the medical provider at the time of the physical examination for their review.

The parent/guardian must complete The Preparticipation Examination, The Athlete with Special Needs: Supplemental History Form (page two), if applicable, for a student with a disability that limits major life activities, and insert the date of the required physical examination on the top of the page.

The licensed physician, APN or PA who performs the physical examination must complete the remaining two pages of the PPE, and insert the date of the examination on the The Preparticipation Examination Physical Examination Form (page three) and The Preparticipation Examination Clearance Form (page four).

The licensed physician, APN or PA must also sign the certification statement on the PPE form attesting to the completion of the professional development module. Each board of education and charter school or nonpublic school governing authority must retain the original signed certification on the PPE form to attest to the qualification of the licensed physician, APN or PA to perform the PPE.
*NOTE*: PLEASE be aware that the school physician must review and approve ALL athletic pre-participation physical examinations. If the pre-participation health history, physical examination and clearance forms are handed in late, the official confirmation for participation on an athletic team can be delayed, regardless of the status determined by your personal physician. Therefore, it is important to obtain the examination in a timely manner. In addition, PLEASE review the forms BEFORE leaving your physician’s office to assure they are completed fully. Any missing information will result in disapproval and will also delay the athlete’s participation.

The school district must provide written notification to the parent/guardian, signed by the school physician, indicating approval of the student’s participation in a school-sponsored interscholastic or intramural athletic team or squad based upon review of the medical report, or must provide the reason(s) for the disapproval of the student’s participation.

For student-athletes that had a medical examination completed more than 90 days prior to the first official practice in an athletic season, the New Jersey Department of Education Health History Update Questionnaire (HHQ) form must be completed, and signed by the student’s parent/guardian. The HHQ must be reviewed by the school nurse and, if applicable, the school’s athletic trainer. HHQ form may be found here.
These forms are fillable PDF forms and may be partially completed PRIOR to printing them. A hard copy with original
signatures must be submitted.
*IMPORTANT*: If your child has received a physical within the 365 days prior to the first official practice, but did not submit the forms at the time of exam, you DO NOT need a new exam. Request your physician to complete the forms for the day of your child’s exam and submit the forms to the school nurse.
*If you are unsure as to the date of your child’s last PPE on record with the school nurse, visit your parent portal in Genesis (termed sports physical exam in Genesis)*
HIGH SCHOOL ATHLETES: submit the completed Preparticipation Physical Evaluation set of forms to the school nurse no later than TWO WEEKS BEFORE the first practice. DO NOT give the packet to your coach.
MIDDLE SCHOOL ATHLETES: submit the completed Preparticipation Physical Evaluation set of forms to the middle school nurse no later than TWO WEEKS BEFORE the first practice. DO NOT give the packet to your coach.
If you are submitting packets DURING THE SUMMER MONTHS whether you are a high school athlete or a middle school athlete, the entire and completed packet must be submitted into the Athletic Department (located in the high school main office) no later than TWO WEEKS BEFORE the first practice.
The first day of practice and the date that registration opens for each sports season on the Genesis parent portal is listed as a pop-up on the athletics page of the district website.
