Please see below the forms that current Faculty and Staff may require throughout the School Year.  

Absence Request Form (Unpaid Absences): Use this form to request an unpaid absence.

AESOP Tip Sheets:  Use this document to assist in submitting or cancelling an absence and obtaining a substitute.

Employee Change Form: Use this form to change any contact or personal information.

FMLA/Leave of Absence Form: Use this form to request extended Leaves of Absences.

Scheduled Absence Procedure:  This document contains all pertinent information related to requesting a scheduled day off including procedures and responsibilities of the staff member.

Tuition Reimbursement Form (Staff): Use this form if requesting reimbursement for courses taken at an accredited college or university for a degree or certification program.

Tuition Reimbursement Form (Administrator): Use this form if requesting reimbursement for courses taken at an accredited college or university for a degree or certification program.

Vacation Carryover Request Form: Use this form if requesting to carryover unused vacation days.
