2.  Please download, print, and complete one of the appropriate residency documents(choose the correct documentation based on personal circumstances as noted in the Student Residency Eligibilty and Enrollment Information Form above):

3.  Please download, print, and complete all the following required documents:

4.  Please  retrieve the following documents:

  • Birth Certificate / Passport / Other Official Document (Age Indicator)
  • Current Universal Child Health Record
  • Legal Custody Documentation (if applicable)
  • Transfer Card (if applicable)
  • Last Report Card (if applicable)
  • Other Assessment Data (if applicable)
  • 3 proofs of actual residency (see Parent / Legal Guardian Certification)
  • Parent / Guardian Photo ID

5.  Please scan and have the following documents ready to upload with online registration:

  • Parent/Legal Guardian Certification
  • Appropriate Residency Affidavit
  • District Request for Records
  • Medical History Report
  • Birth Certificate / Passport / Other Official Document (Age Indicator)
  • Current Universal Child Heath Record / Immunization Record
  • Legal Custody Documentation (if applicable)
  • Transfer Card (if applicable)
  • Last Report Card (if applicable)
  • Other Assessment Data (if applicable)
  • 3 proofs of actual residency (see Parent / Legal Guardian Certification)
  • Parent / Guardian Photo ID

6.  Please use the link to our portal below to register and upload all required documents; registration cannot be completed until all documents are submitted.

If additional support is needed for uploading documents, please call 732-613-4006 ext 2216 to schedule an appointment with Central Registration Office.

Visit the evening onsite evening student registration, support, and transition service on Tuesdays and Thursdays from 5:30 - 7:30 PM at South River Middle School.  South River Middle School is located at 3 Montgomery Street in South River, NJ 08882.  Additional support will be provided with the online registration portal.
