Residency
South River Public Schools monitors residency requirements for all students. Questions related to residency requirements can be directed to the district's residency officer, Kasie Rosario. Ms. Rosario can be reached in the residency office at (908) 239-2590 or through email at krosario@srivernj.org.
Families are to provide updated residency information whenever there is an address change.
Student Eligibility & Enrollment Form
Please download, print, and complete one of the appropriate residency documents (choose the correct documentation based on personal circumstances as noted in the Student Residency Eligibility and Enrollment Information Form above):
- Homeowner Affidavit
- Landlord Affidavit
- Sublease Affidavit
- Affidavit Student-Parent Affidavit
- Affidavit Student-Resident Affidavit
Families must also provide three (3) proofs of residency and a photo ID. The following will be accepted as proof of residency:
If you own this residence: email or provide complete copies of:
Deed to property in your name or
Official property tax bill in your name, and/or
Mortgage statements including your name and this address
If you are renting this residence: email or provide
Complete copy of lease or
Landlord Affidavit form from your landlord or
Sublease Affidavit from responsible person other than landlord who is providing this home to you
If you do not own and do not rent this residence, check and provide:
Homeowner Affidavit

property tax bill showing your name and this address
Sewer bill showing your name and this address
Cell phone bill showing your name and this address
driver’s license / state-issued ID
canceled check

bank statement
military records
divorce judgment / property settlement agreement
medical records
